Keep your strength as the weight comes off. How to prevent GLP-1 muscle loss

October 24, 2026 Medically reviewed by Dr. Ivan Bracic, DC (2026-10-05)
Dr. Ivan Bracic, DC, explaining three things to do before starting a GLP-1 medication

You can help limit GLP-1 muscle loss with strength training, protein at every meal, and a plan before your first dose. This guide covers semaglutide and tirzepatide, how much protein to aim for, and strength training for seniors you can start this week.

Do GLP-1 medications cause muscle loss?

Yes, some of the weight lost on a GLP-1 is muscle. In the STEP 1 semaglutide trial, 38% of the weight lost was lean mass, per a 2025 joint advisory. Plan for your muscle from day one, alongside the medication your doctor chose.

The advisory comes from four groups, including the American Society for Nutrition and The Obesity Society. Because muscle is about half of lean mass, it estimates roughly 20% of the weight lost was muscle itself.

I'm Dr. Ivan Bracic, DC, founder of Novaré Injury Care and Rehab, and I've cared for 30,000+ patients across Illinois and Florida. I don't prescribe GLP-1s, but you should feel respected, listened to, and clear on your options. My part is the exercises and eating habits that protect your strength.

Why GLP-1 muscle loss happens

GLP-1s turn down your appetite, so most people eat less food and often less protein. The 2025 joint advisory says faster weight loss, deeper calorie cuts, and skipping strength training all raise lean mass loss. Watch for that mix once your appetite drops.

Some people lose more muscle than others. The advisory names older age, menopause, lower testosterone, inactivity, and no strength training as risk factors. Check yours early, since it's part of preventing a more serious issue down the road.

A 2024 review in Diabetes, Obesity and Metabolism found lean mass loss varies widely between trials. Some show about 15% or less of weight lost, and others 40% to 60%. Lean mass also includes organs, bone, and water, so not all of it is muscle.

What GLP-1 muscle loss means for strength and balance after 50

Muscle gets you up stairs, out of chairs, and steady after a stumble. Muscle mass drops about 3% to 8% per decade after age 30, and faster after 60, per a review on muscle and aging. Keep that in mind, since a GLP-1 can add to that loss.

So treat the scale as one number among several. After 50, your strength, your balance, and how easily you move matter just as much as your weight.

If you feel unsteady, add balance exercises for seniors to your strength work.

5 ways to protect muscle on a GLP-1

These are the five steps I recommend, backed by the sources in this article.

  1. Tell your prescribing doctor muscle matters to you. Ask how they'll track your strength and body composition, not just your weight. Never change a dose on your own.
  2. Strength train at least three days a week. The joint advisory recommends this plus 150 minutes of moderate aerobic activity weekly, matched to your fitness level.
  3. Eat protein first at every meal. It helps when appetite is low, but the advisory warns protein alone likely won't preserve muscle without strength training.
  4. Stay hydrated and rest well. The advisory links nausea and vomiting on GLP-1s to dehydration. Rest days help you keep training week after week.
  5. Track strength, not just the scale. Note how easily you rise from a chair or climb stairs, and share changes with your doctor.

What to do before starting a GLP-1 medication

If you and your doctor decide on a GLP-1, get a strength baseline first and start training before the first dose. The 2025 joint advisory recommends a baseline exam of muscle strength, function, and body composition. Ask your prescribing doctor how they'll measure yours.

For older adults, the advisory names simple tests like sit-to-stand, stair climbing, and the timed up-and-go. I recommend writing down how you do on a chair stand and a flight of stairs, so you can compare later.

Whether to start, which medication, and what dose are decisions for you and your prescribing doctor. Bring questions about side effects or lab work to that doctor too.

How much protein do you need on a GLP-1?

Usually more than a quiet appetite asks for, so plan it instead of waiting to feel hungry. The joint advisory says targets of 1.2 to 1.6 grams per kilogram a day have been proposed during weight loss. Ask your prescribing doctor or a registered dietitian to set your exact number.

For context, the general adult recommendation is 0.8 grams per kilogram, and a kilogram is about 2.2 pounds. The advisory also offers a simpler goal of 80 to 120 grams a day, which may be easier to follow.

A high-protein breakfast for muscle when your appetite is low

The joint advisory lists fish, eggs, Greek yogurt, cottage cheese, beans, and nuts among protein-rich foods, so eat the protein first and the rest after.

  • Eggs with vegetables left over from last night's dinner.

  • Cottage cheese or Greek yogurt with fruit or a few nuts.

  • Leftover fish or chicken in a warm breakfast bowl.

  • Beans or lentils with an egg on top.

  • A protein shake on days you can't face food, a backup the advisory says some people use.

Strength training on a GLP-1 to keep muscle

Strength training gives your body a reason to hold on to muscle while you lose fat. The review on muscle and aging found progressive resistance training builds muscle and strength even in frail older adults. Start with chair squats, wall push-ups, and band rows.

Some people feel more tired on a GLP-1, so start light and add a little each week. My guide on how to build muscle after 60 lays out a full routine.

Eat a protein-rich meal after you train.

Signs of muscle loss to watch for

Muscle loss is gradual, so watch your everyday tasks and raise these changes with your prescribing doctor:

  • Standing up from a chair takes your hands or a second try.

  • Stairs, grocery bags, or jar lids feel harder than they used to.

  • You feel less steady on your feet or have a near-fall.

  • Your workouts stall, or you lift less than you did last month.

  • Tiredness beyond what you expect, which the joint advisory lists as a possible sign of a nutrient shortfall.

What a muscle and strength evaluation at our clinic includes

We don't prescribe or manage GLP-1s; we help you keep your strength. Expect a first visit that typically runs 1 to 1.5 hours, mostly with me.

  1. Your history and goals. Your medications, your activity, and what you want to keep doing, such as golf or keeping up with grandkids.
  2. A hands-on exam. I check strength, balance, and how you rise, walk, and climb, with digital X-rays on site when your exam calls for them.
  3. A strength plan. Home exercises plus in-clinic rehabilitative exercises and therapy built around your energy and your joints.
  4. Support for weak muscles. For some patients we add EMSCULPT NEO functional wellness, a supervised muscle therapy used alongside exercise, never instead of it.
  5. Medical oversight. We're a medically integrated practice with an MD medical director. Questions about your medication go to your prescribing doctor.

What patients say about strength care at our clinic

My experience at the office was well worth the visit. The Dr. Was informative, experienced, but most importantly, leveled with me. I didn't feel like he followed a script which made him appear more personable in my experience. I scheduled with the office twice then cancelled cause I didn't think I'd be impressed as far as my personal expectations but eventually proved wrong. Would I recommend this office? Yes! This office has a great team that's on point and the Dr. Is able to deduce the issues you may be facing or have faced and help you shift in the direction you'll need in order start or continue improving your mobility and the quality in your every day enjoyments. I give this a 4 even though they may deserve a 5 because I'm only going off my first visit.

GLP-1 side effects that need prompt medical care

MedlinePlus, the National Library of Medicine's guide to semaglutide, says to call your doctor right away or get emergency care for symptoms like these. Check your own medication's guide with your pharmacist too.

  • Ongoing stomach pain that may spread to your back, with or without vomiting.

  • Swelling of the face, mouth, tongue, or throat, or trouble breathing or swallowing.

  • Less urination, or swelling in your legs, ankles, or feet.

  • Yellowing of your skin or eyes, or pain in your upper stomach.

  • A rapid or pounding heartbeat, or changes in your vision.

  • A lump or swelling in your neck, hoarseness, or trouble swallowing.

Frequently Asked Questions

Can you regain muscle lost on a GLP-1?

Often, at least in part, with strength training and enough protein. The aging review found progressive resistance training builds muscle and strength even in frail older adults. Start small, stay consistent, and judge your progress over months.

How do I stop losing muscle on Ozempic or another GLP-1?

You can't stop all of it, but you can help limit it. The joint advisory pairs strength training at least three times a week with enough protein. Build both habits now, and track your strength along with your weight.

Can I build muscle while on a GLP-1?

It's possible for many people, though it's harder while you're eating less. The aging review shows resistance training builds muscle even in older adults. Make your exercises a little harder over time and eat protein after each session.

Should I lower my GLP-1 dose if I'm losing muscle?

Don't change your dose on your own. The joint advisory calls for checking muscle strength and body composition, and dose decisions belong to your prescribing doctor. Bring your strength notes to that appointment.

Is it safe to see a chiropractor while on a GLP-1 if I'm nervous about going?

For most people, yes, when care starts with a proper exam and your full medication list. In one Fort Myers review, a patient says they were nervous about going to a chiropractor at first. Tell us about your GLP-1 and any dizziness before treatment starts.

What exercises and eating habits keep muscle on a GLP-1?

Strength training three days a week, daily walking, and protein first at every meal. The joint advisory recommends at least 150 minutes of moderate aerobic activity weekly alongside strength work. Pick habits that will help keep you strong for years.

Do I need protein shakes on a GLP-1?

Not always, but they can help on low-appetite days. The joint advisory says some people meet their protein targets with high-protein shakes or bars. Choose whole foods first and keep a shake as a backup.

Does EMSCULPT NEO replace exercise on a GLP-1?

No. We use it as a supervised therapy alongside exercise, never as a substitute. Our functional wellness page describes it as an adjunctive therapy for muscle re-education. Keep up your strength training at home either way.
