Cervical or Lumbar? Where the Nerve Is Pinched Decides the Plan.

A pinched nerve is a nerve root being compressed or irritated where it leaves the spine, most often by a herniated or bulging disc, a narrowed opening (foraminal stenosis), or swelling after an injury. In the neck it is called cervical radiculopathy: pain, tingling, or weakness that runs into the shoulder, arm, or hand, often after a car accident or whiplash. In the low back it is lumbar radiculopathy, the pattern most people know as sciatica.

Arm symptoms usually trace to C5, C6, or C7. Leg symptoms usually trace to L4, L5, or S1. Exam findings plus imaging tell us which nerve root is involved before any treatment begins.

It depends on what is pinching it. Disc-related nerve compression responds to non-surgical spinal decompression and Cox flexion-distraction. Joint and soft-tissue irritation responds to adjustments, manual therapy, and Class IV laser. The exam and imaging decide which, so we do not guess.
Stop doing what aggravates it, keep the neck or back in a neutral position, and get the level diagnosed so treatment targets the right nerve. Most patients notice a change in the first few visits once decompression and inflammation control begin. Same-day appointments are available at both clinics.
Carpal tunnel syndrome, shoulder impingement or rotator cuff problems, piriformis syndrome, hip joint problems, and peripheral neuropathy can all mimic radiculopathy. That is why we examine reflexes, strength, and sensation and image the spine before treating.
Many cases improve within a few weeks of conservative care and most settle within 6 to 12 weeks. Longer-standing compression, larger disc herniations, or nerve involvement after a car accident can take longer. Your plan is re-evaluated as you progress.
On your back with the neck supported in a neutral position, or on your side with a pillow that keeps the head level and a pillow between the knees for a lumbar nerve. Avoid sleeping on your stomach with the head turned. We review positions with you at the first visit.
Repeated bending, twisting, and lifting for a lumbar nerve; looking down at a phone for long stretches or sleeping on your stomach for a cervical nerve. Pushing through sharp radiating pain, numbness, or weakness makes it worse. New weakness or loss of bladder or bowel control needs urgent medical care.
No referral is needed. The first visit is $200 and includes the exam, digital x-rays, and a report of findings. He moved to Fort Myers in 2019 and opened his Injury Care and Rehab practice there in 2021.
